Correction Notice from Olympus:
Problem
Olympus has received a complaint that upon using Olympus bronchoscope model BF-1TH190 with an endotracheal tube, the tip became lodged (entrapped) inside the endotracheal tube connector. The investigation results revealed that the incident occurred due to the endoscope’s insertion tube outer diameter being too large for the connector. Selection of an endotracheal tube where the size is incompatible with Olympus endoscopes can result in delayed procedures, failure to ventilate the patient and/or endoscope damage.In an effort to maximize patient safety and mitigate any potential risk to patient health, Olympus is providing the following information related to using an endotracheal tube:
• Before inserting the endoscope with an endotracheal tube into the patient, slide the tube along the entire length of the endoscope insertion section to confirm that the insertion section of the endoscope can be inserted smoothly into the endotracheal tube. If it cannot be inserted smoothly, the covering material of the bending section of the endoscope or the external surface of the insertion section may be damaged. When using a lubricant, make above confirmation before applying the lubricant. In addition, to aid in the selection of the proper size of the endoscopes please refer to the Addenda attached which provide information on the maximum insertion tube outer diameter.

Our records indicate that your facility has purchased one or more of the affected endoscopes. Olympus requests you to take the following actions:
1. Carefully read the content of this letter and addenda attached. The addenda provide information on the maximum outer diameter of the insertion tube for the affected endoscopes.
2. Ensure all personnel in all areas of the hospital that use the affected endoscopes are completely knowledgeable and thoroughly aware of the content of this letter and addenda attached.
